Endoscopic Mini Facelift

Minimal incision, natural lifting

For patients whose facial sagging has begun but who are not yet ready for a full facelift. A ~1.5 cm incision within the temporal scalp minimizes scarring, and the endoscope visualizes the facial nerve directly for safe deep-plane dissection.

1. What is an Endoscopic Mini Facelift?

The Endoscopic Mini Facelift uses approximately 1.5 cm incisions at 4 points within the temporal scalp (5 points in male patients with M-pattern hair loss as an exception) for endoscope insertion to lift the mid-face (cheek, nasolabial area).

Because the incisions are hidden inside the hairline, scars are virtually invisible. Compared to a traditional facelift, recovery is faster and the procedure delivers an effective result for early to moderate facial sagging. Most importantly, the endoscope allows the surgeon to directly visualize the facial nerve and other critical structures during dissection, dramatically improving safety.

Core Advantages

Nerve protection (the biggest advantage)

When incisions are small, dissecting without direct visualization is dangerous. The endoscope provides direct visualization of the facial nerve and other critical nerves, dramatically reducing the risk of nerve injury.

Incisions hidden in the hairline

Incisions are made within the temporal scalp, so scars are virtually invisible from the outside.

Fast recovery

Less tissue trauma means less bruising and swelling — most patients return to daily life within 2 weeks to 1 month.

2. Comparison with Traditional Facelift

Both procedures have their merits; the right choice depends on the patient's degree of sagging and overall condition.

Item Endoscopic Mini Facelift Facelift (Deep Plane)
Incision ~1.5 cm within temporal scalp (hidden) Temporal · pre-auricular · post-auricular extended
Dissection plane Deep plane (limited skin excision) Deep plane + sufficient skin excision
Best for Early to moderate sagging Moderate to severe sagging
Recovery ~2 weeks to 1 month ~3 weeks to 3 months
Lifting effect Strong but limited by skin excision Strong and definitive improvement
Surgery time ~2–3 hours ~4 hours (6–7 hours when combined with neck lift)

Strengths and Limitations of the Mini Facelift

The Endoscopic Mini Facelift actually dissects all the way into the deep plane, so the lifting effect itself is strong. The key difference is that the small incision limits how much skin can be excised.

When there is significant sagging, lifting the tissue produces excess loose skin. Because the mini facelift cannot excise enough skin, it cannot be pulled aggressively. In short: the power of deep-plane dissection is preserved, but skin excision is limited.

When the Mini Facelift Is Not the Best Choice

If sagging is already advanced (deep nasolabial folds, collapsed jawline, accompanying neck sagging), excess loose skin must be excised — which the mini facelift cannot do sufficiently.
In those cases, an Extended Deep Plane Facelift is the better choice. The accurate decision should be made through consultation with the surgeon.

Accurate Diagnosis at ROI

At ROI Plastic Surgery, we evaluate sagging extent, skin elasticity, and bony framework comprehensively to determine whether a mini facelift is sufficient or a full facelift is required.

  • No over-surgery: We never recommend more surgery than necessary.
  • Accurate indication: We clearly distinguish patients who only need a mini facelift from those who need a full facelift.
  • Long-term planning: Treatment plans consider not only your current condition but also future aging.

3. Who Is It For?

🌿

30s–40s with early sagging

Patients in their 30s and 40s who notice early aging changes but do not yet have severe sagging — managing the early stage prevents accelerated progression.

👤

Early nasolabial folds & cheek descent

Deepening nasolabial folds and downward-drifting cheek volume that make the face look longer. Focused mid-face lifting restores a younger impression.

⏱️

Short downtime needed

Patients who cannot afford a long recovery period due to work or daily commitments — mini facelift offers faster return to normal life.

Looking for a natural change

Patients who want a subtle, natural improvement rather than a dramatic transformation — perfect for refreshing facial impression without an obvious surgical look.

4. Surgical Method

Nerve protection — the endoscope's greatest safety advantage

Small incisions inherently mean limited direct visualization, which can be dangerous during dissection.
Through the endoscope, we directly visualize the facial nerve and other critical structures,
dramatically reducing the risk of nerve injury and enabling safe surgery.

Deep-plane dissection + SMAS lifting

The Endoscopic Mini Facelift actually dissects into the deep plane.
The SMAS (Superficial Musculoaponeurotic System) is lifted and anchored for a natural, durable result.
Note that limited skin excision means severe sagging cases may need a different approach.

🔧 Fixation Method — Suture + Endotine Ribbon

Lifted tissues are stably anchored using a combination of sutures and the Endotine Ribbon. Endotine Ribbon is an FDA-cleared, biocompatible absorbable material that naturally dissolves within the body in approximately one year, so no removal procedure is required.

Temporal scalp incision — minimal scarring

Incisions are made approximately 1.5 cm within the temporal scalp,
hidden inside the hairline and virtually invisible from outside.
Meticulous closure further optimizes scar management.

5. Recovery Guide

Recovery time may vary depending on individual health and constitution.

D+1

Day 1 after surgery

Mandatory dressing visit. Swelling and bruising are usually mild compared to a full facelift. Cold compresses and prescribed medications manage the early course.

D+12

Day 12 after surgery

Sutures within the scalp are removed (the appointment can be adjusted between D+10 and D+12 depending on schedule). By this point most major bruising and swelling have subsided, and light cleansing and makeup become possible.

2w

2 weeks after surgery

Most patients can resume daily life and social activities. Residual mild swelling is manageable with makeup.

Final

Final Result

Over 2 weeks to 1 month, residual swelling resolves and the lifted contour settles into a natural, refreshed appearance.

Note: Bruising, swelling, and changes in sensation vary by individual. A return-to-activity plan tailored to your occupation and lifestyle will be discussed during consultation.
